Saccharomyces boulardii has useful evidence for selected diarrhea contexts, especially antibiotic-associated diarrhea, but probiotic effects are strain- and situation-specific.
Cochrane pediatric AAD review: Lactobacillus rhamnosus or S. boulardii at 5-40 billion CFU/day appeared most appropriate among studied probiotics.
Trials commonly use specific S. boulardii preparations during antibiotic courses or acute diarrhea episodes; ordinary yeast products are not equivalent.
Avoid in severe immunocompromise, central lines, critical illness, or high-risk hospital contexts unless a clinician specifically recommends it.
